We do not collect medical uploads, identity documents, payment card details or detailed medical history through these website forms.

Why we need it

To acknowledge your request, contact you about appointment confirmation and respond to general enquiries. Marketing consent is not bundled into appointment processing.

Retention

Proposed retention for web enquiry records is 90 days after receipt unless a documented operational or legal need requires longer retention. The practice will confirm the final period covering email copies, backups and failed queues.
